Issue: On certain vehicles, a software problem could cause the lower beam headlamps not to turn on as it becomes dark outside if the multi-function switch is in the "OFF" or "PARKING LAMP" position. This could also cause the daytime running lamps (DRLs) to turn off when the vehicle is moving. As a result, the vehicles may not meet certain requirements of Canadian regulations. Safety Risk: Headlamps and/or DRLs that don't turn on could reduce visibility, make the vehicle less visible to others and increase the risk of a crash. Issue: This recall is for aftermarket replacement rear window glass produced by Vitro Automotriz. Certain rear window glass may not have been properly tempered. If the window breaks, the glass may separate into large pieces instead of very small ones. Note: This is a voluntary recall that is not being conducted under the requirements of the Motor Vehicle Safety Act. Safety Risk: Glass that breaks in to large pieces could create a risk of injury. Corrective Actions: The company will notify consumers by mail and instruct them to inspect the designation code marked on the rear window glass. Certain vehicles may be equipped with rear seat belt retractors that do not comply with the requirements of Canada Motor Vehicle Safety Standard (CMVSS) 208 - Occupant Protection in Frontal Impacts. The automatic-locking retractors (ALR) of the rear seat belts may not switch to a locked position when the seat belt is fully pulled out of the retractor. On certain vehicles, a defect in the ignition lock cylinder could cause the key to stick in the "start" position if the vehicle interior temperature is sufficiently high. If the vehicle is operated in this condition and the interior temperature cools or the vehicle goes off-road or is subjected to some other jarring event, the key could move out of the "start" position, rotate past the "run" position and into the "accessory" position. If this were to occur, engine power, power steering and power braking would be affected, increasing the risk of a crash causing injury and/or damage to property. The timing of the key movement out of the “run” position, relative to the activation of the sensing algorithm of the crash event, may also result in the airbags not deploying in a subsequent collision, increasing the risk of injury. Certain vehicles equipped with an Automatic Transfer Case may have a condition in which an electric signal short may cause the transfer case to switch to neutral without operator input. The transfer case mode selection knob would still indicate the last selected drive mode, but the driver would not have any feedback that the transfer case is in neutral. If the transfer case switches to neutral while the vehicle is in motion, the driver would experience a loss of motive power and, if the vehicle is parked without the parking brake applied, the vehicle could roll away. On certain vehicles, the interface between the electrical terminals of the Heating, Ventilation, and Air Conditioning (HVAC) blower relay resistor module and the wiring connector (which powers the module) may be incapable of conducting higher current levels for sustained periods of operation. In addition, moisture and other contaminants (such as road de-icing materials) may enter the fresh air intake plenum and contact the internal circuit of the module or corrode the terminals. These conditions may cause the relay resistor module or wiring connector to overheat, causing various symptoms such as: the HVAC blower may not function on certain or all blower speed settings; a burning-plastic smell or smoke may be present in the vehicle, or; the HVAC blower may run continually with the ignition in the OFF position. These warning signs could, in certain instances, be leading indicators of an impending vehicle fire, which could result in property damage and/or personal injury. Correction: Owners will be provided with additional warranty coverage for the blower motor resistor module/wiring connector overheating condition. Under this "special warranty coverage", should this condition occur within 10 years or 240,000 km from first vehicle registration, dealers will install a replacement blower motor resistor and resistor module connector.

On certain vehicles, the rear axle cross pin was improperly heat treated (during the manufacturing process) and could fracture and become displaced within the rear differential. As a result the driver may hear a banging noise emanating from the rear axle. Also, if the pin shifts out of position it could create an interference and cause the rear axle to lock. On certain vehicles equipped with a Heated Washer Fluid System (HWFS), a short circuit on the printed circuit board for the washer fluid heater may overheat the control-circuit ground wire. A repair was implemented in a recall in 2008, however, there have been new reports of thermal incidents on heated washer fluid system modules after this improvement was installed. This situation may cause some minor distortion, but also could lead to melting of the plastic around the heated washer fluid system, or cause a fire. Correction: Dealers will permanently disable and remove the heated washer fluid module. An updated page will be provided and inserted in the Owner Manual to document the removal of the feature.

On certain vehicles equipped with an 8.1L engine, the crankshaft position sensor may fail. If the sensor operates intermittently, the Service Engine Soon (SES) light may illuminate and the vehicle may run rough. In addition, the engine may stall, and if so, may re-start immediately or after a cool-down period. If the sensor becomes inoperable permanently, the engine will quit running and will not re-start. On certain vehicles, the 2nd row centre occupant seat belt routing may make it difficult to position the lap portion of the safety belt low around the hips of occupants. A lap portion of the seat belt routing that is not low and snug on the hips can allow unfavourable occupant kinetics in the event of a crash. Moreover, the higher routing allows the lap belt to ride up on an occupant's abdomen instead of fitting low around their hipbones, and therefore, can expose them to more risk of abdominal and internal organ injury. Correction: Dealers will modify the guide loop.

Certain vehicles were built with fuel module reservoir assemblies that contain fuel pump wires and/or connectors that may overheat under certain operating conditions. Fuel pump wires that overheat may become exposed and result in one or more conditions; 1) If the ignition circuit wire is exposed and shorts to ground, the fuel pump fuse will blow, disabling the fuel pump and causing an engine stall or no-start condition. 2) If sufficient heat is conducted to the pass-through connector, a hole in the connector may result, which may cause the Service Engine Soon light to be illuminated during the emission system diagnostics routine. Fuel vapour (and in some limited cases liquid fuel) may leak out of the fuel tank through the hole in the connector body. 3) If the ignition circuit or ground wire is exposed and shorts to the fuel level sender card wires, inaccurate fuel level readings may result. Correction: Dealers will affect repairs.

Certain vehicles equipped with an automatic transmission fail to comply with the requirements of CMVSS 101 - Location, identification and illumination of displays. Under certain vehicle starting conditions, the shift lever position indicator located in the instrument panel cluster may not illuminate. In subsequent vehicle start-ups, this condition may or may not occur again. If the shift lever position indicator does not illuminate, the driver may not know which gear the vehicle is in and the vehicle may move in an unintended direction, resulting in possible injury to others outside of the vehicle. Correction: Dealers will reprogram the instrument panel cluster.

On certain vehicles, the hydraulic brake booster's pressure accumulator may crack and/or separate from the Hydro-Boost assembly during normal vehicle operating conditions. If a separation occurred and the hood of the vehicle was open, fragments from the accumulator could cause injury to people in the immediate area. In addition, the presence of this crack or fractured surface could allow the hydraulic fluid to leak from the accumulator circuit of the booster assembly. The loss of fluid would cause increased steering and braking effort. Correction: Dealers will test and, if necessary, replace the Hydro-Boost assembly.

Certain vehicles located in Ontario, Quebec, New Brunswick, Nova Scotia, Prince Edward Island, and Newfoundland & Labrador may experience unwanted Antilock Brake System (ABS) activation and increased stopping distances during low-speed brake application (less than 16 km/h and greater than 6 km/h). This condition does not set any ABS codes nor does it illuminate the ABS warning lamp. Correction: Dealers will remove the wheel speed sensor and thoroughly clean the wheel speed sensor mounting surface on the bearing, apply Zinc-X to the cleaned surface, grease the mounting surface, reinstall the wheel speed sensor, and check the output voltage to ensure the wheel speed signal is within specifications.

On certain vehicles, the fuel rail crossover tube retainer screws were not manufactured to specifications and may break. If a tube retainer screw breaks, fuel could leak from the fuel rail crossover joint, the engine may stall and, if an ignition source were present, an engine compartment fire could occur. Correction: Dealers will inspect the fuel rail crossover tube assembly and, if necessary, replace the two tube retainer screws.

On certain vehicles, the relief valve o-ring seal within the brake hydro-boost module could fail. If this happens during braking applications, the driver may be able to hear an engine compartment noise similar to the sound that occurs when the steering wheel is turned to a full stop position. The driver could also experience a slight increase in steering efforts while braking and parking. Under certain driving conditions, a slight increase in the applied brake pedal effort may be required to achieve the same vehicle deceleration rate as prior to the seal failure. Correction: Dealers will inspect the hydro-boost module, and replace it, if necessary.

On certain 2500 series vehicles, a tear could develop in the mid-frame cross member at its attachment to the left frame rail during a 30 degree left angle frontal impact of sufficient severity. If the mid-frame cross member tears during frontal impact and produces a sharp edge, contact of the sharp edge with the fuel tank may result in a puncture of the fuel tank and possible fuel leakage. If a sufficient amount of fuel were to leak out and if an ignition source were present, a vehicle fire could occur. Correction: Dealer will install a fuel tank shield.

Certain vehicles fail to conform to CMVSS 209, seat belt assemblies. These vehicles have a condition where one or two sensors in the driver's and front seat passengers seat belt retractors may be inoperative. The seat belt retractors will lock when the belt webbing is extracted during a crash; however the mechanism that locks the seat belt retractor when the vehicle decelerates quickly, such as during heavy braking, may not restrain the occupant as intended during a crash, and could result in injury to the occupant. Correction update: None required as no effect on vehicle safety. See recall 04-158.

Certain vehicles have an internal component in the 2nd and 3rd row outboard seat belt retractor that could be cracked. With repeated actuation of the locking mechanism, the crack could spread to the point where the seat belt would no longer lock. In a crash, increased occupant injury could result from the no-lock condition. Correction: Dealers will inspect the outboard seat belts for specific lot numbers and replace the belts if necessary. GM is enclosing an inspection procedure for the owners to use or they can have their dealers do the inspections.

On certain Yukon Denali and Yukon XL Denali vehicles, the positive battery cable assembly that could come in contact with the steering shaft universal joint. This could eventually cause a wear through of the cable assembly convoluted conduit and cable insulation and result in intermittent electrical shorting. This could result in overheating and igniting of the cable insulation and conduit and a possible underhood fire. Correction: Dealers will inspect the positive battery cable for proper routing and reroute if necessary. Cable will also be inspected for wear of the conduit and cable insulation, and replaced if necessary.

On certain vehicles, the body cross sill may contact the front, right-hand brake pipe. This could eventually wear a hole in the brake pipe over the normal life of the vehicle and result in a brake fluid leak.
